Why do I get intermittent FSC 3016 - drive load failed messages?

    Cause: Among other things, intermittent fault system code (FSC) 3016 can be caused by using an unsupported SCSI bus configuration. An unstable SCSI bus can disrupt the communication to and from the library/drives, which results in incomplete processes (that is, the movement of media).

    Solution:

    Avoid the following:
    -- RAID SCSI host bus adapters and interfaces
    -- Over-extended or unnecessary bus lengths per the SCSI specification
    -- Daisy-chaining too many devices on a single SCSI bus
    -- Putting hard disk drives and tape drives on the same SCSI bus
    -- Lack of or improper use of termination
    -- Substandard or damaged SCSI cables/terminators
